Neurodegenerative processes alter neuronal and glial physiology and cause cognitive and mnemonic impairments. Aim of this PhD thesis is to investigate the involvement of the cholinergic system and the role of mTOR pathway in the mechanisms of memory encoding in the hippocampus and to study the pathophysiological processes at the base of the cognitive impairments in different experimental models of neurodegeneration: in particular normal brain aging, neuroinfiammation and chronic cerebral hypoperfusion. These mechanisms are studied focusing on the morpho-functional alterations in the neuron-astrocytemicroglia triad.

Brain diseases currently affect one in six people worldwide; they include a wide range of neurological diseases, from Alzheimer’s and Parkinson’s diseases to epilepsy, brain injuries, brain cancer, neuroinfections, and strokes. The treatment of these diseases is complex and limited due to the presence of the blood–brain barrier (BBB), which covers the entirety of the brain. The BBB not only has the function of protecting the brain from harmful substances; it is also a metabolic barrier and a transport regulator of nutrients/serum factors/neurotoxins. Knowing these characteristics when it comes to the treatment of brain diseases makes it easier to understand the lack of efficacy of therapeutic drugs, resulting from the innate resistance of the BBB to permeation. To overcome this limitation, drug delivery systems based on nanotechnology/microtechnology have been developed. Brain-targeted drug delivery enables targeted therapy with a higher therapeutic efficacy and fewer side effects because it targets moieties present in the drug delivery systems.

Central nervous system (CNS) tumors are a heterogeneous group consisting of more than 100 tumor types originating from the brain, cerebellum, brain stem, spinal cord, and meninges. Despite the positive developments of recent years, especially in the treatment of high-grade brain tumors, treatment outcomes have not yet reached an acceptable level. However, due to advances in clinical and experimental studies, the genetic and molecular structures of many high-grade CNS tumors are now better understood. The World Health Organization’s 2021 Classification of CNS Tumors has aided the understanding of the genetic, molecular and immunological structures of these tumors as well as their interactions with their microenvironment. Targeted therapies and vaccines developed with immunotherapy protocols are promising developments. This book explores the natural history of CNS tumors and discusses relevant treatment protocols.

After decades of focusing on how to alleviate and prevent recurrence of acute CNS injuries, the emphasis has finally shifted towards repairing such devastating events and rehabilitation. euromodulation is among the fastest-growing areas of medicine, involving many diverse specialties and affecting hundreds of thousands of patients with numerous disorders worldwide. It can briefly be described as the science of how electrical, chemical, and mechanical interventions can modulate the nervous system function. A prominent example of neuromodulation is deep brain stimulation (DBS), an intervention that reflects a fundamental shift in the understanding of neurological and psychiatric diseases: namely as resulting from a dysfunctional activity pattern in a defined neuronal network that can be normalized by targeted stimulation. The application of DBS has grown remarkably and more than 130,000 patients worldwide have obtained a DBS intervention in the past 30 years—most of them for treating movement disorders.
