Aim of this PhD thesis is to investigate the involvement of the cholinergic system and the role of mTOR pathway in the mechanisms of memory encoding in the hippocampus and to study the pathophysiological processes at the base of the cognitive impairments in different experimental models of neurodegeneration: in particular normal brain aging, neuroinfiammation and chronic cerebral hypoperfusion. textformat="05"><div>Brain diseases currently affect one in six people worldwide; they include a wide range of neurological diseases, from Alzheimer’s and Parkinson’s diseases to epilepsy, brain injuries, brain cancer, neuroinfections, and strokes. The treatment of these diseases is complex and limited due to the presence of the blood–brain barrier (BBB), which covers the entirety of the brain. The BBB not only has the function of protecting the brain from harmful substances; it is also a metabolic barrier and a transport regulator of nutrients/serum factors/neurotoxins. Knowing these characteristics when it comes to the treatment of brain diseases makes it easier to understand the lack of efficacy of therapeutic drugs, resulting from the innate resistance of the BBB to permeation. To overcome this limitation, drug delivery systems based on nanotechnology/microtechnology have been developed. Despite the positive developments of recent years, especially in the treatment of high-grade brain tumors, treatment outcomes have not yet reached an acceptable level. However, due to advances in clinical and experimental studies, the genetic and molecular structures of many high-grade CNS tumors are now better understood. The World Health Organization’s 2021 Classification of CNS Tumors has aided the understanding of the genetic, molecular and immunological structures of these tumors as well as their interactions with their microenvironment. Targeted therapies and vaccines developed with immunotherapy protocols are promising developments. Whether these advances comprise large-scale approaches, such as The Cancer Genome Atlas, which provides a modern classification of cancers based on molecular profiles, or genealogy initiatives, which seek to trace the movement of our early ancestors out of Africa, genomic technology has taken us closer to developing targeted therapies and a refined understanding of our evolutionary journey. It is against this backdrop that we summarized some of the recent advances in the field of precision medicine, or personalized medicine, as they pertain to neurosurgery.
