When an emergency exceeds the usual response capacity, medical care can no longer be provided under ideal conditions. Time becomes limited, resources grow scarce, and every decision may determine who receives care first, which intervention takes priority, and how available resources can be used responsibly. This book addresses precisely such circumstances: medical practice in emergencies and disasters from a clinical, public health, and human-centered perspective. Throughout its chapters, it integrates the foundations of risk management, territorial and social vulnerability, the main hazards affecting Ecuador, and lessons learned from historical disasters, linking them to the preparedness and competencies healthcare professionals need to respond effectively to critical situations. The text progresses from understanding disasters as a public health issue to the direct care of critically ill patients. It covers triage, systematic assessment, life support, resuscitation, trauma and polytrauma, management of life-threatening conditions, analgesia, and sedation, together with the recognition and management of neurological, respiratory, cardiovascular, infectious, gastrointestinal, metabolic, and toxicological emergencies. Rather than merely describing diseases and procedures, the book proposes a way of thinking and acting when needs exceed available resources. Prioritizing, stabilizing, adapting, and making decisions thus become essential competencies. With particular attention to the Ecuadorian context and resource-limited settings, this book seeks to transform knowledge into response capacity by preparing professionals to recognize risks, make evidence-informed decisions, and provide comprehensive care when every minute, every resource, and every decision matters.
